Extracorporeal Shock Wave Lithotripsy (ESWL) part of the non-invasive method of Nephrolithiasis. Nephrolithiasis is a disease that has a very high recurrence rate, the recurrence rate reaches 60% after 5 to 10 years. Preliminary study at Dr. Hospital. Saiful Anwar as many as 16 ESWL patients with 31.25% supine position felt pain from the side of the energy source. ESWL procedure is accompanied by pain, the direction of the energy source of the shot greatly affects the pain. Pain scale assessment to determine appropriate pain management. The purpose of this study was to determine the difference in pain scale of ESWL patients in the supine position, the direction of the energy source being shot from the side and below. Analytical observational research design with a cohort approach. Observations were made when the ESWL sample was 20 respondents with purposive sampling technique. The research instrument used the Visual Analugue Scale (VAS) observation sheet. The results of the study were mostly 56 - 5 years old (35%), male 65%, high school and college education 40%, private employment 50%, and marital status 85%. The mean of the pain scale in the supine position with the direction of the energy source being shot from the side 1.5 and below 1.1 there is a difference of 0.4. Wilcoxon statistical test at pulse rates of 5 (0.564), 10 (0.132), and 12 (0.114). Descriptively, in this study, there is a difference in the pain scale of ESWL patients with Nephrolithiasis in the supine position with the direction of the energy source being shot from the side and below, while statistically there is no relationship. Each increase in pulse rate needs to be assessed on a pain scale, for the effectiveness and success of ESWL
